To request a certificate of Baptism, First Communion, Confirmation or Marriage, complete the form below. Please note the following:

• You may only request your own certificate, or for your child under age 18.

• All certificates must be picked up at the church office in person, with valid ID. We cannot mail, email or fax certificates to you personally.

• Out-of-town requests will be mailed to your local Catholic parish, where you can pick them up in person with valid ID.

• Please provide the parish name, address and phone number for delivery.

• Please contact the parish office at 209-823-7197 ext. 0 or email your request at receptionist@st-anthonys.org It can take a few days to research the hand-written Sacrament records.
